Starting with your own. Providence is calling Medical Social Workers. We're seeking a compassionate, caring licensed social worker to join our home care & hospice team. This individual evaluates patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by the Clinical Manager through patient record documentation, verbal communication and direct observation. Position Specifics. This position is located at Providence SoundHomeCare and Hospice in Olympia, WA. This is a full time (40 hours per week), day shift, benefits eligible position. REQUIREMENTS Education: A Master's degree in social work from an accredited school of social work is required. Licenses/Certifications: Washington State Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) is preferred. LICSW or LASW – Associate may be considered. Valid Washington State driver?s license along with an insured, reliable vehicle, CPR certification and Mantoux testing is also required.
